"Questionnaire, 1st registration of the health professions"

Shown below in alphabetical order are five original four-page questionnaires required of Jewish medical professionals in Lviv (Lwów, Lvov, Lemberg) by the Nazi German occupation authorities. Each is stamped with the Star of David, and three are stamped "JUDE." I obtained them from dealers in the U.S. and Canada in 1999 and 2003.

• Dr. Herman Frenkel, age 46 (b. Aug. 26, 1895, Lwów) signed March 8, 1942; Ausweis #109 issued April 1, 1942 

• Dr. Eugeniusz Frisch, age 43 (b. June 10, 1898, Limanowa) signed Sept. 30, 1941; Ausweis #112 issued April 1, 1942 

• Dr. Febus Geisler, age 29 (b. March 10, 1913, Lwów) signed March 10, 1942; Ausweis #122 issued April 1, 1942 

• Dr. Efroim Halpern, age 32 (b. Feb. 4, 1910, Zelechów Wielki, Kamionka Strumiłowa region) signed March 9, 1942; Ausweis #166 issued April 1, 1942 

• Dr. Sindel Segal, age 47 (b. March 23, 1895, Zbaraż, Tarnopol region) signed March 9, 1942; Ausweis #384 issued April 1, 1942
